New Patient Forms

The Fertility Center's electronic medical records system allows you to complete new patient forms on line prior to a first appointment. Our front office staff will provide a User ID number as well as a Password that gives you access to the patient portal of eIVF through this website. In addition to streamlining paperwork, eIVF lets you have access to appointment schedules, cycle instructions and lab results, including semen analyses.

If you can’t complete the on-line registration process beforehand, please plan to arrive 30 minutes prior to your scheduled appointment time to enter your information at the computer workstation we’ve set up for patients in a private room just off our lobby.

When you arrive at our office, you will be asked to sign a statement confirming your insurance information as well as a financial agreement granting our offices permission to file your insurance. If you require surgery, you will also need to sign a location disclosure statement.  These forms are listed below so you can print at home and bring them with you to our office, if you prefer:
